Health Insurance Dental Insurance Vision Insurance Paid Time Off 401(k) matching The primary purpose of the Patient Navigator is to ensure the safe discharge and satisfaction of resident's discharged from the facilities.
This position also assists in assigned areas and is involved with direct day-to-day operations of the organization as assigned in accordance with current Federal, State and local standards, guidelines and regulations that govern long-term care facilities.
The Patient Navigator reports to the Administrator of their designated facilities (North Westchester Region:
Sutton Park, North Westchester Restorative, and Long Island Care Center).
Scope of work:
Track all discharges to home and hospitals at assigned facilities and determine if patient/family were satisfied with facility services.
For patients retuning home, meet with them prior to discharge (if possible) and explain follow up services, as well as the Navigator's role.
If patients are satisfied, assist them in posting a Google or other internet review, if not already done so by guest relations or other department.
For patients that have returned home, attempt to contact them or a family member to see if any assistance is needed in maintaining their safe return home.
If resident needs assistance with aftercare arrangements such as doctors' appointments, home care, equipment issues, etc.

, we will assist them remotely if possible.
Communicate any concerns and issues to appropriate facility personnel, and follow-up to ensure resolution.
If the resident is not doing well at home and wishes to return to the facility, provide them with assistance for readmission to the facility when appropriate.
Provides reports to management summarizing patient outcome and satisfaction, as assigned.
Identifies areas of improvement, trends, and through QAPI process works with team on improving areas of concern.
Collaborates with all departments to improve areas identified.
Attends pertinent facility meetings such as Discharge meeting to participate in the assigned areas and contribute to the operational success of assigned facility.
As role evolves in facility, undertakes additional management and oversight responsibilities as assigned by the Administrator.
Assist marketing team with their efforts.
Specifically, act as the community liaison for each facility in this region hosting one community event per quarter per site.

Salary:
$23.
50/hr Shift:
Part-time or Full-time.
Estimated Salary: $20 to $28 per hour based on qualifications.
