Competitive Rates Contract Providers must be licensed and or certified by the NYS Education Department in one of the fields designated below.
Speech Pathology Occupational Therapy Special Education Physical Therapy All contract providers must possess a NYSDOH approval letter to provide EI services.
Contract providers must maintain and provide to Just Kids D&T, a copy of the providers General and Professional Liability Insurance naming Just Kids Diagnostic and Treatment Center, Inc.
as additionally insured, with $1,000,000 minimum coverage per occurrence:
and must properly complete, file and provide Just Kids D&T with the following:
a W-9 form, a copy of your professional license, NPI number and proof of current physical with immunizations including; Tdap(every 10 years), MMR, Varicella, PPD, and Covid series.
